IntroductionCommunity nursing is important in caring for patients in a holistic approach. Using chronic disease self-management (CDSM) and the transtheoretical lifestyle change model (TTM), nurses can ensure care is specific to the patient, which can increase compliance with their new program. By involving patients as much as possible in their own care, they can reclaim some of the power that health care can take away from them. By self-managing their chronic illness, patients can live fuller lives than if they depended on the healthcare team to plan and implement care. An example of a patient who uses CDSM is CG, a close family friend of Nicole LeSage. CG suffers from rheumatoid arthritis.BodyC.G. is a 58-year-old woman who lives in Vancouver, British Columbia with her husband. CG's daughter, a doctor, also suffers from rheumatoid arthritis and CG's husband has suffered from leukemia for two years. Given that the entire family deals with (chronic) health issues and the rest of the family lives in the interior of British Columbia, Nicole's family is close to CG's, as her mother Nicole grew up with CG and now they have become a support system as well. CG has difficulty focusing on the positive aspects of his health. However, she is willing to try anything to change her negative mindset about her health in order to improve the health of her family. As the entire family faces health issues on a daily basis, their outlook on health is bleak. She was misdiagnosed by two doctors and tried numerous medications to control her symptoms, but nothing worked for her.
