For the sake of this assignment, I will have to use the term “client” very loosely for this population. One interaction I can think of is with DB, an 8-year-old girl who was diagnosed with oppositional defiant disorder (ODD). The SDG, as described in our text, is characterized by a pattern of defiant, hostile, and negativistic behaviors (Lambert, 2005; AACAP, 2009). For DB, she often displayed behaviors towards others that were vindictive and malicious in nature, she threw tantrums at the drop of a hat, frequently acted inappropriately by deliberately doing things that annoyed others, and constantly tested boundaries , as well as being hypersensitive or touchy about others' comments. His behaviors disrupted his daily performance in various contexts, particularly in his home environment, in his social interactions with his peers when playing, and in an educational setting. If I were an occupational therapist working with DB, there would be intervention strategies I would do. focus on because of their unique needs, as well as their noted strengths and interests. One strategy I would use for DB would be to incorporate a therapeutic board game like the one discussed in our text “The Game of Talking, Feeling, and Doing” (Lambert, 2005). This game would focus on a skill development format that would include structure through predetermined rules, have an outlet for unexpressed feelings, begin to establish a sense of confidence regarding sharing information with others, and promote discussion of appropriate problem solving. Another strategy would include observation and interactions in one's home environment, where a room would focus more on the parents and children, and an observation would be made regarding their dynamics and interactions with the environment. As an occupational therapist working with DB in his home environment, suggestions on coping strategies may be recommended in efforts to improve DB's maladaptive behaviors and increase parental interactions by providing suggestions for setting limits, model, structure and routine.2.
