    FINDINGS: Severe chest pain on the right side. The __ reported friction. Lateral PM showed questionable mediastinal lymphadenopathy. The patient had no cough or sputum production of ___. ___'s CT scan showed the possibility of a 1 x 2 cm right paratrachial lymph node. Several smaller anteromediastinal lymph nodes all measuring less than 1 cm were also evaluated. The patient was treated with Indocen with relief of her chest pain. The ___ level was 64.5 with a ___ 50. PPD was negative, controls were positive. CT scan of the abdomen showed normal liver, spleen, pancreas, kidneys, adrenals, uterus, left ovarian cyst, large gallstone at the neck of the gallbladder, no lymphadenopathy and its mammogram was within normal limits. The patient reports a band-like pain around her right chest, with changes in pain accompanied by coughing and deep breathing. There is no change in position. The patient reports increased shortness of breath with exercise over the past 5 months. No nausea, vomiting, fever, chills, night sweats, abdominal pain, urinary problems, kidney stones, diarrhea, ___ melena, headache, visual disturbances or rash. PAST MEDICAL HISTORY: Unremarkable. PAST SURGICAL HISTORY: Notable for tubal ligation. NO KNOWN DRUG ALLERGIES. MEDICATIONS ON ADMISSION: Indocin 25-50 mg poq8 hours and Anaprax 270 mg pobidThe patient rarely drinks alcohol and smokes regularly. PHYSICAL EXAMINATION: Weight 231 pounds, temperature 98.6, heart rate 60, respiratory rate 24, blood pressure. 110/70. The HEENT exam was unremarkable. The neck was supple, without masses, lymphadenopathy or sounds. The lungs presented with small, diffuse wheezes, but otherwise without abnormality. Cardiovascular examination revealed regular, normal S1 and S2 rate and rhythm. A slight questionable change in S1 with the __ maneuver and a questionable 1/6 systolic ejection murmur is best heard at the lower border of the sternal. The abdomen was obese, had bowel sounds in all quadrants, soft and non-tender. It was difficult to tell if there was organomegaly. The extremities showed no clubbing, cyanosis, or edema. Neurologically, the patient was normal. HOSPITAL COURSE: The patient was admitted for ____ evaluations as well as for a consultation with the pulmonology department. The pulmonology department recommended that the patient undergo a bronchoscopy which was performed.
