    They argue that government involvement, through policymaking, is necessary to achieve essential elements of health system reform , and identified six attributes of an ideal health care delivery system: (1) availability of clinically relevant patient information both to all providers at the point of care and to patients themselves through the electronic health record systems; (2) active management through appropriate coordination among multiple providers; (3) accountability between providers and other healthcare professionals through meaningful collaboration for the reliable delivery of high-quality, high-value care; (4) provider competence and responsiveness to patient needs and easy patient access to appropriate care and information, including after hours, through multiple system entry points; (5) clear accountability for total patient care; (6) continuous innovation and learning to improve the quality, value, and patient experience of healthcare delivery. The Commonwealth Fund has, however, highlighted that leadership is an essential element of health system delivery, efficiency and success.
