  • Essay / Direct Admit/Inter-Facility Transfer Conversion...

The Hospital Operations Resource Center (HORC) has several responsibilities involving indirect patient care; one of them being bed management. Bed management is responsible for admitting patients from different entry points to the hospital. Bed management supports all direct admissions from either the doctor's office or inter-facility hospital transfers. If the doctor calls to admit someone directly, a direct admission form must be completed with the information obtained from the doctor. If the patient is coming from another facility, the information will be obtained from the nurse caring for the patient. The information on this paper form includes: the patient's name, social details, date of birth, age, gender, date and time of arrival, contact details of the admitting doctor, diagnosis, patient contact information, source of admission, bed type and status; and if the patient is coming from another facility, inter-facility transfer information as well as insurance information must be completed. This document must be accompanied by a physician order sheet with a telephone order obtained from the physician regarding bed type and status, such as admitting the patient to a cardiac telemetry bed as an inpatient. This information is then entered into the site’s electronic dashboard. and an admission request is chosen so that this information can be sent to the receiving unit. The next step is to fax this form along with the order to the receiving unit and the Patient Access Center (PAC). The PAC is responsible for actually admitting the patient into the Meditech system so that nurses can trace the patient and doctors can enter orders. Then a phone call is made to these units to ensure they have received the fax...... middle of paper......steps required to construct the document and manage challenges along the way. Mandatory components will provide more accurate information and avoid major errors. Finally, the comparison between the two is that electronics provide better workflow, thereby increasing efficiency.
