The results of this research study may be used in reports, presentations and publications, but the researcher will be careful not to identify the subjects. Information obtained solely for research purposes will be stored in research records, will not affect the subjects' current medical care, and will not be included in their medical records. Type of identifiable information collected: Informed consent forms and subject information forms (which have contact information) require the names of the research subjects. The original paper versions of these forms will be kept in a locked cabinet in a locked room in the Adaptive Neural Systems Laboratory. No further copies will be produced. Privacy Protection and File Security: All other forms will contain only the subject code and will not contain any identifiers. These forms include some that we prepared specifically for this study (to record data during different sessions) and standard forms such as those used to record adverse events that were provided by the Office of Research Integrity of the FIU. Hard copies of each of these completed forms will be securely stored in the subject file in the Adaptive Neural Systems Laboratory. Efforts will be made to keep subjects' personal information private and confidential. However, PHI may be disclosed if required to do so by law and/or after obtaining separate authorization from the subject giving us his...... middle of paper ...... online or by mail electronic. Information about sensitive or illegal behavior: We do not intend to collect data regarding sensitive or illegal behavior. A certificate of confidentiality will not be required. Protected Health Information: All medical history information will be limited to information that the subjects themselves wish to disclose during screening procedures. This information will be kept in their subject files (in a locked filing cabinet in a locked room in the FIU Adaptive Neural Systems Laboratory), with only subject codes used as identifiers. Database or archival data If archival data from other studies are to be used for comparison, then only anonymized data with subject codes as identifiers will be used. All other laboratory studies follow the same confidentiality format as described here..
